Esperanza y Vida, meaning Hope and Life in Spanish, is a culturally responsive health education and patient navigation program that helps Latino communities access preventive health care and cancer screening services.
The program works with Latino individuals and families in both rural and urban communities to increase awareness of breast cancer, cervical cancer, and diabetes while reducing barriers to care. Through education, one-on-one support, and community outreach, Esperanza y Vida empowers participants to take charge of their health and connect with lifesaving preventive services.
Esperanza y Vida provides:
Health education on breast cancer, cervical cancer, and diabetes prevention
Assistance scheduling recommended health screenings
Patient navigation to help individuals understand and access the health care system
Bilingual translation and interpretation support
Referrals to health care providers and community resources
Guidance through follow-up care and treatment when needed
Our trained patient navigators are trusted, culturally competent professionals who help individuals and families navigate every step of the health care process. They work closely with patients and providers to ensure people understand their options, overcome barriers to care, and receive the support they need.
Esperanza y Vida is built on a community health worker model that has been proven to effectively reach underserved populations. The program is led by bilingual staff and trained Latino community health advisors, including both men and women, as well as breast cancer survivors who share their experiences to educate and inspire others.
